Louisiana Payment Areas
When Provider Enrollment assigns a Medicare provider number the
payment locale must be determined for Medicare payment. Medicare
payments are based on a payment schedule:
- Medicare Fee Schedule (MFS)
The Louisiana Medicare Fee Schedule (MFS) has 2 payment
locales: 001 and 99.

All Parishes not listed in payment 001. |
If you have office locations in both locales, a
CMS 855 application for each is needed. |
